  library_spec/FORM_NAME=form_name

  Specifies  a  form  from  a  library.  The qualifier must immediately
  follow  the  library name. You specify the name of the form that  you
  wish  to  edit  after  an  equals  sign.   The presence of /FORM_NAME
  changes the default input file type to FLB.
